Report Covid Symptoms, Exposure, or a Positive Covid Case

Please fill out and submit your respective form below if any of the following are true:

1. You are currently experiencing COVID like symptoms such as:

• 失去味觉或嗅觉

• 大量盗汗

• Cough
• 头疼
• 喉咙痛

2. You have tested positive for COVID in the last 10 days (even if you are experiencing 无症状)
   
   
3. In the last 10 days, you have been in close physical contact with a person who is confirmed to have tested positive for COVID or who has any symptoms consistent with 新型冠状病毒肺炎?

隔离指南

如果你进行测试 积极的，不论是否接种疫苗，你必须:

• Notify the Covid 支持 Team by filling out a Covid支持表单
• 隔离至少5天
• The Covid 支持 Team will clear you to return to work or school after your 5-day 以下情况的隔离期: 
• symptoms are not present (or are resolving) AND  
• no fever is present for at least 24 hours (without the use of medication)

• 隔离 may extend to 10 days or longer if:

Quarantine Guidelines for Close Contacts Exposed to a Someone Who Has Covid

A “亲密接触” is defined as any of the following people who were exposed to a person while they 都感染了新型冠状病毒肺炎.

• A person who has shared the same indoor airspace with an infected person for 15 minutes 或者超过24小时
• OR a person who was within 6 feet of an infected person for 15 minutes or longer over 任何室内环境下的24小时
• OR A person who had unprotected contact with the infected person’s body fluids and/or secretions, for example, being coughed or sneezed on, sharing utensils or saliva, or providing care without using appropriate protective equipment

If you DO NOT 有症状吗? 都必须:

• Test as soon as possible and/or within 3-5 days after your last exposure to determine 你的感染状况.

• Wear a highly protective mask when around others, both indoors and outdoors, for 10 暴露后整整几天

  • If symptoms arise during this time, get tested immediately, go home while you await your test results, and fill out a Covid支持表单

• 在家至少呆5天
• Wear a highly protective mask when around others, both indoors and outdoors, for 10 暴露后整整几天
